RECOMMENDED ACTION(S):

SUMMARY:
Approval to execute Contract No. MA-042-19010990 with Conduent Healthy Communities Corporation for customized web-based information system will support Public Health’s www.ochealthiertogether.org website, which hosts health-planning resources, including more than 300 local health-related indicators.
BACKGROUND INFORMATION:
In 2014, the Health Care Agency (HCA) developed and launched www.ochealthiertogether.org which includes: 1) more than 300 real-time health indicators for community health planning and research, 2) progress trackers allowing Orange County health indicators to be compared to national state and local data and goals, 3) a database of over 2,000 promising practices related to health issues, 4) a central repository for local reports, 5) quick linkage to resources via a live link to 211OC and 6) the ability to customize screens to promote and highlight local planning and initiatives. These resources assist non-profit organizations, hospitals, local government, residents, academic institutions, and other partners in identifying and developing strategies to address health needs - often at a zip code or city level. In addition the site publicly hosts the plans and activities of the Orange County Health Improvement Partnership, which promotes collaborative and strategic work among public and private partners to address selected health priorities and disparities.
Between July 1, 2018, and June 30, 2019, the website received 129,806 page views and had 42,059 users. The website has approximately 4,000 users per month.

The www.ochealthiertogether.org website was developed under a sole source contract with Healthy Communities Institute (HCI) Corporation in June 2014. The original contract was executed administratively, as it was under the threshold and did not require Board of Supervisor approval. The contract was amended in 2015 to include Business Association language and on June 23, 2015, your Honorable Board of Supervisors (Board) approved Amendment No. Two which added two additional enhancements to the original contract in the amount not to exceed $259,100, renewed the Contract for two more years, effective November 1, 2015, through October 31, 2017, approved the Master Agreement for Sponsorship of the platform and authorize HCA to accept funding in support of the platform, and authorized HCA to execute individual Sponsorship Agreements and accept funding in support of the previous action. Those enhancements supported the creation and licensing of 85 locally-selected indicators that can be viewed at the sub-county level such as cities, zip codes, school districts, etc.; and the creation of a web-based profile of Orange County older adults containing information and analysis on 79 different indicators of health, social and economic well-being. Previous sponsors of the platform have included Alzheimer's Orange County, First 5 Orange County - Children and Families Commission of Orange County, and Orange County United Way. HCI has delivered all materials by deadlines and maintained the website as stated in the contract.
On September 26, 2017, the Board approved the final Contract renewal for the fourth and fifth years with Healthy Communities Institute Corporation for customized web-based information system; effective November 1, 2017, through October 31, 2019, in an amount not to exceed $200,000. On January 2018, Healthy Communities Institute Corporation changed their name to Conduent Healthy Communities Corporation to become an independent publicly traded company. This is a new three-year contract term with the same sole source vendor.
The contract is a sole source contract and a completed Sole Source Request Form is attached to this Agenda Staff Report.
The Contractor's performance has been confirmed as at least satisfactory. HCA has verified there are no concerns that must be addressed with respect to Contractor's ownership/name, litigation status or conflicts with County interests.
This contract does not have any subcontracts or pass through to other providers. See Attachment B for Contract Summary Form.
HCA is requesting the Board approval to execute this Contract with Conduent Healthy Communities Corporation for Customized Web-Based Information System, effective November 1, 2019, through October 31, 2022, in an amount not to exceed $300,000 as referenced in the Recommended Action. The annual cost of $100,000 includes annual licensing fees to support the platform and existing indicators and additional professional services based on identified need.
HCA is also requesting your Board's authorization to accept funding support from member organizations of the Orange County Health Improvement Partnership to support the website in an aggregate amount less than or equal to the actual cost of the website.
HCA requests that your Board approve the Master Agreement for Sponsorship of the Conduent Healthy Communities Corporation platform, and authorize the HCA Director, or designee, to accept funding from, and execute individual Sponsorship Agreements with, Orange County Health Improvement Partnership member organizations in support of the Conduent Health Communities Corporation platform as referenced in Recommended Actions #2 and #3.
FINANCIAL IMPACT:
Appropriations for this Agreement are included in the HCA’s FY 2019-20 Budget and will be included in the budgeting process for future years.
