RECOMMENDED ACTION(S):

 

Authorize the County Procurement Officer or authorized Deputy to execute Amendment No. 2 to amend Contract with Safety Net Connect, Inc. for Whole Person Care Connect platform, under the amended terms, conditions, pricing and scope of work, effective September 10, 2019, through December 31, 2020, non-renewable, in an amount not to exceed $532,400 for a revised cumulative total of $2,953,650.

 

 

 

 

SUMMARY:

 

Approval to execute Amendment No. 2 to amend the Contract with Safety Net Connect, Inc. for the Whole Person Care Connect platform will allow real-time notification and coordination of coordinated care plans with Whole Person Care Community Partners, as required by the California Department of Health Care Services.

 

 

 

BACKGROUND INFORMATION:

 

On June 28, 2016, your Honorable Board of Supervisors (Board) approved the submission of an application to the California Department of Health Care Services (DHCS) to implement a Whole Person Care (WPC) Pilot Program targeting Medi-Cal beneficiaries that are experiencing homelessness. The overarching goal of the WPC Pilot is the coordination of health, behavioral health and social services, as applicable, in a client-centered manner with the goals of improved beneficiary health and wellbeing through more efficient and effective use of resources. In November 2016, the Health Care Agency (HCA) was notified that the application was approved and an agreement was executed with DHCS.

 

On June 27, 2017, the Board approved Contract No. MA-042-17011762 with Safety Net Connect, Inc., to develop and implement the WPC Program electronic care coordination platform called "WPC Connect." This platform allows for the opportunity to collect and share client data among participating WPC community-based entities such as hospitals, community clinics, recuperative care providers and various HCA programs. WPC Connect supports ongoing case management and monitoring of the WPC enrollees, as well as ongoing evaluation for strategic improvements in the WPC Pilot program. Amendment No. 1 was executed administratively, effective October 23, 2018, to include the State Business Associate language and revise a few elements of the scope of work.

 

WPC Connect went live in November 2018 and has 30 participating entities to date. As a pilot program, and in accordance with the guidance from DHCS, the WPC Program undergoes a constant evaluation of all activities, including the WPC Connect platform, through a process called "Plan, Do, Study, Act," and therefore is in a constant state of implementation, evaluation, and improvement. As of today, the care coordination platform provides for an electronic shared record of services that each WPC enrollee is receiving, which is accessible to the enrollee's entire care team. This includes the ability for members of the care team to receive notifications any time there is activity with one of their clients. Standard information for each enrollee includes their name and address, as indicated on their Medi-Cal application, contact information for the enrollee's primary care provider and, if applicable, the contacts for case managers and/or Behavioral Health providers. Referrals can be made through the system to current WPC Providers, including recuperative care. HCA's authorization of recuperative care admissions and extended stay requests are also done through WPC Connect.

 

An update to the WPC Connect system will be rolled out in September 2019 and will include improvements for both hospitals and non-hospital providers to make electronic referrals to recuperative care and eliminate follow-up faxing of information, the ability for providers to see the number of recuperative care beds each provider has available for referrals, the ability for WPC providers to send secure messages to other WPC providers and the functionality for shelter bed providers to indicate the number of beds available for referrals from hospitals.

 

On May 21, 2019, the Board ratified the submission of a WPC Round Three proposal to DHCS for additional WPC funding. Included in this proposal was funding for enhancements to the WPC Connect platform. On May 31, 2019, HCA was notified that its proposal was approved.

 

The proposed Amendment No. 2 includes the enhancements approved by DHCS, which are as follows:

·       Development of an Application Programming Interface hub that will make it easier to develop ways to integrate and exchange data in a known and standard way and will help with linkages to other databases such as the Community Referral Network.

·       Develop an assessment module accessible to all WPC participating entities, which will add standard assessment tools to the front-end intake as an option for use.

·       For recuperative care providers and willing community shelter providers, develop the ability to indicate the number of beds they have available and to receive referrals via WPC Connect from hospitals, community clinics and other WPC participating entities.

·       Develop the Outer-circle/Close-the-loop referral process that would allow the ability to “eRefer” a patient from an inner-circle organization (any of the WPC participating entities on WPC Connect) to a trusted outer-circle network for that organization (common referrals to community-based organizations that are not on WPC Connect) through an electronic process using work queues and functionality that supports the updating of the activities.

·       Additional Professional Services/Feature Development for any services that may be determined on an as-needed basis or that fall outside of Scope of work. Since the WPC Program is a pilot program, the evolution of WPC Connect is tied to input from the WPC participating entities as the enhancements are developed and rolled out. As a result, there may be proposals for additional enhancements and functionality that occur as the above and current work is in process.

 

Due to the limited amount of time remaining to use WPC dollars to fund continued enhancements and new functionality for WPC Connect, HCA and Safety Net Connect, Inc. have agreed that the work on the above enhancements needs to begin as soon as possible; therefore, this proposed amendment is coming before the Board with the proposed effective date commencing upon Board approval. This Contract does not currently include any subcontractors or pass through to other providers. See Attachment B for Contract Summary Form.

 

HCA has verified there are no concerns that must be addressed with respect to Contractor's ownership/name, litigation status or conflicts with County interests.

 

HCA requests that the Board authorize the County Procurement Officer or authorized Deputy to execute Amendment No. 2 to Contract No. MA-042-17011762 with Safety Net Connect, Inc., as referenced in the Recommended Action.

 

 

 

FINANCIAL IMPACT:

 

Appropriations for this Contract are included in Budget Control 042 FY 2019-20 Budget and will be included in the budgeting process for future years.
