RECOMMENDED ACTION(S):

 

 

1.

Approve the non-financial Memorandum of Understanding between the Social Services Agency, Health Care Agency, County Executive Office – Office of Care Coordination, OC Community Resources, Orange County Information Technology, Orange County Sheriff-Coroner Department, Probation Department and OC Public Works for the establishment of Homeless Adult and Family Multi-Disciplinary Personnel Teams, to allow the agencies to share information with the Homeless Adult and Family Multi-Disciplinary Teams.

Authorize the Department Head or designee of each agency, listed in Recommended Action #1, to execute the Memorandum of Understanding.

 

3.

Authorize the County Executive Office – Office of Care Coordination, or designee, to make minor modifications to the Memorandum of Understanding in accordance with the requirements of California Welfare and Institutions Code Section 18999.8 to allow for the designation of additional Homeless Services Provider Agencies not currently identified in the Memorandum of Understanding as members of the Homeless Adult and Family Multi-Disciplinary Personnel Teams; to establish data sharing and integration mechanisms as applicable; and to integrate language related to subsequent changes to State laws and regulations as applicable.

 

 

 

 

 

SUMMARY:

 

Approval of the non-financial Memorandum of Understanding for the establishment of Homeless Adult and Family Multi-Disciplinary Personnel Teams will allow the agencies to share information with the Homeless Adult and Family Multi-Disciplinary Teams and will facilitate and expedite the identification, assessment and linkage of homeless individuals to housing and supportive services within the County of Orange.

 

 

 

BACKGROUND INFORMATION:

 

The Office of Care Coordination, in partnership with Social Services Agency, Health Care Agency, OC Community Resources, Orange County Information Technology, Orange County Sheriff-Coroner Department, Probation Department and OC Public Works, is requesting the Board of Supervisors’ (Board) approval of a non-financial Memorandum of Understanding (MOU) between the various County departments for the establishment of the Homeless Adult and Family Multi-Disciplinary Personnel Team (MDPT).  The MOU will provide a framework for the participating agencies to share certain confidential information with, and only for use by, the MDPT pursuant to the State of California (State) Assembly Bill 210 (AB 210) which added Section 18999.8 to Welfare and Institutions Code (WIC), and create a platform for facilitating the expedited identification, assessment and linkage of homeless individuals to housing and supportive services within the County of Orange (County) through this information sharing.  AB 210 created the ability for county departments to share confidential information to ensure continuity of care for those individuals and families experiencing homelessness, and with the Board’s approval of the MOU, the County will be able to implement and operate a MDPT. 

 

Additionally, the MOU will further the interests of the County, in a collaborative effort, to meet the individual and collective objectives of the participating agencies as outlined in AB 210.  Sharing confidential information with the MDPT pursuant to the MOU will facilitate and expedite the achievement of the following goals: reduce duplication of efforts and services by the participating agencies; expedite the identification, assessment, and linkage of homeless individuals and families to housing and supportive services within the County; allow the participating agencies to share certain confidential information with the MDPT to coordinate housing and supportive services to ensure continuity of care; improve the quality of care and outcomes for homeless individuals and families; maximize access to and usage of appropriate federal and State services and benefits; increase efficiency of processes and services; reduce recidivism; and more effectively serve high utilizers of services.

 

The Board has directed the County Executive Office to pursue a contract with a consultant to assist in the development of a Scope of Work that may be used to develop a Request for Proposal for the creation of a System of Care Data Integration System.  This MOU provides the foundation for the governance structure ultimately needed for the System of Care Data Integration System by identifying the necessary departments that will participate in the data-sharing platform based on their regular interaction and engagement with those experiencing homelessness and high-utilizers of county systems, which will be the targeted population in the first cohort of the data integration project. This MOU allows the MDPT to operationalize data sharing and determine the use cases necessary in ensuring the continuity of care for individuals experiencing homelessness.  This information will be utilized in the work with the aforementioned consultant as well as the design of the System of Care Data Integration System. Additionally, this MOU outlines the data sharing capabilities and limitations in compliance with AB 210 to provide a framework for confidentiality, roles and responsibilities needed to facilitate the work with the consultant and eventually the implementation of the broader System of Care Data Integration System.

 

Under the proposed MOU, participating agencies are permitted to disclose certain information elements identified in the MOU to the MDPT unless another State or federal law prohibits the disclosure of the information by the participating agency.  The Office of Care Coordination will provide oversight and coordination of activities under the MOU and will coordinate the participating agencies’ joint development and implementation of uniform policies and procedures that pertain to and support the MOU.  Orange County Information Technology (OCIT) will provide support and consultation for the participating agencies related to data sharing and integration activities under the MOU.  The MOU also serves as the AB 210 protocol for the County’s MDPT and will be submitted to the State Department of Social Services, as required by WIC Section 18999.8(e).
