Adopt 2017 Retiree Self-Funded PPO Health Plan Rates for Wellwise and Sharewell; authorize Chief Human Resources Officer and designee to continue to apply for Medicare Part D federal subsidy under Medicare Prescription Drug, Improvement and Modernization Act of 2003 for Wellwise retiree health plan - All Districts

Adopt 2017 Retiree Insured HMO and PPO Health Plan Rates; authorize Chief Human Resources Officer or designee to execute 2017 Group Service/Benefit Policies with Anthem Blue Cross, Kaiser Permanente Health Plan, Inc. and SCAN Health Plan after approval by State Department of Managed Care or Centers for Medicare and Medicaid Services and report back to the Board if any changes are required - All Districts

Supervisor Nelson - Direct Registrar of Voters to place "An Ordinance of the County of Orange, California Repealing Section 1-2-9 of the Codified Ordinances of the County of Orange and Adding Article I, Section 104 to the Charter of Orange County Imposing a Lifetime Limit on the Number of Terms for Members of the Orange County Board of Supervisors" on the ballot for Statewide General Election on November 8, 2016; direct County Counsel to prepare ballot language and impartial analysis of ordinance; and request Auditor-Controller to review measure and prepare a fiscal impact statement (Continued from 7/12/16, Item S29A)
