SUMMARY:
Approval of the selection of and Contract with Dayle McIntosh Center for the Disabled to administer the Dignity at Home Fall Prevention program will assist with reducing the number of debilitating falls suffered by older adults and persons with disabilities by providing services such as fall prevention education, in-home assessments, home modifications and minor home repairs.
BACKGROUND INFORMATION:
The Dignity at Home Fall Prevention Program (DHFP) is a preventive service developed to provide resources to at-risk older adults and individuals with disabilities by identifying and addressing factors that lead to falls within the older adult and disabled community. At-risk older adults are defined as individuals who have sustained a fall in the past or are susceptible to a fall due to a disability, prior injury, or the use of mobility aids such as a walker or cane. The service is also extended to individuals with disabilities who are not consider older adults, i.e., individuals under the age of 59 who choose to live independently. Therefore, this service is critical in preserving older adult independence and prevents premature institutionalization. According to research conducted by the California Department of Aging (CDA), one out of five falls in older adults result in a serious injury, which in most cases have been determined to have underlying preventable causes. Services rendered under DHFP are formulated to specifically prevent and eliminate such incidents from occurring.
On May 17, 2022, OC Community Resources/Office on Aging (OCCR/OoA) released a Request for Proposal (RFP) #012-2239805-HRC with an application due date of June 15, 2022. The RFP was advertised in the OC Register two days prior to the release date and advertised through BidSync.com. All qualifying and interested organizations were encouraged to apply via BidSync.com. A proposal was received from only one applicant. The applicant was evaluated and scored by a qualified evaluation panel on the following criteria: Applicant’s Experience and Scope of Services; Qualifications of Personnel; Cost Effectiveness; and Proposed Program Services, Methodology, and Comparability to RFP Objectives. Attachment A consists of the Performance Rating Summary and Individual Scoring Sheets. The evaluation panel recommended award of contract to Dayle McIntosh Center for the Disabled (DMC).
DMC is an established non-profit organization, with expertise in providing services to the disabled community. Their mission bolsters inclusion, which is fostered by the provision of services by disabled adults to other disabled individuals to help promote self-sufficiency within the disabled community. DMC offers a comprehensive support plan for older adults that includes education, resources, the opportunity to connect with others encountering similar difficulties and techniques on how to solve such problems. Moreover, DMC practices an inclusive and demographically representative hiring policy, which imparts to their inclusivity/peer-based model where disabled individuals facing barriers connect with others who are also disabled and have successfully navigated similar barriers.
Board of Supervisors (Board) approval of Contract No. MA-012-23010033 (Contract) with DMC will allow at-risk older adults and disabled individuals access to services such as educational materials on fall prevention, referral to community-based resources, availability of in-home environmental assessments conducted to prevent injuries from falls and criteria-based assessment of older adults that includes the areas of behavioral, environmental and physical health. DMC will ensure that any educational materials will be translated into the County approved threshold languages. DHFP’s holistic approach administered with the inclusivity model implemented by DMC, will assist in diminishing barriers faced by frail older adults and disabled individuals in the areas of fall prevention.
Installations refers to the installment of fall preventative equipment in the homes of at-risk older adults such as grab bars, toilet seat risers, handheld showers, licensed electrician services, unsafe flooring repairs and toilet assist equipment. Additionally, approval of the Contract with DMC will provide 50 basic installations, 15 intermediate installations, three advanced installations and 38 in-home assessments, which will aim to assist a total of 76 unduplicated individuals. Furthermore, DMC will factor in COVID-19 guidelines and ensure that representatives follow all safety protocols when completing home assessments or equipment installations.
The Contract was not submitted
to the Board 30 days prior to contract implementation, due to the significant
amount of time required to complete the RFP process and the budget formulation
for the contracts. Due to this delay, OCCR/OoA was not able to file the item in
accordance with the Board-directed procedure.
Under the second recommended action, OCCR/OoA is requesting delegated authority to execute future amendments to the Contract that will help OCCR/OoA more efficiently administer additional funding, when the state unexpectedly allocates additional funds to County, and respond to state mandated changes to the County’s contracts with its subrecipients or to the program’s operations.
OCCR/OoA is also requesting
delegated authority to execute future amendments to the Contract that are not
required by CDA but will help OCCR/OoA more efficiently manage the contract and
program. Under the third recommended
action, the requested authority would allow OCCR/OoA to amend the Contract
terms without returning to the Board, provided the changes are non-financial,
scope of work and program specific, administrative, and deemed to be in the
best interest of the County.
The County has verified there are no concerns that must be addressed with respect to the Contractors' ownerships/names, litigation status or conflicts with County interests. This contract does not currently include subcontractors or pass through to other providers. See Attachment B for Contract Summary Form. The Orange County Preference Policy is not applicable to this contract award.
FINANCIAL IMPACT:
Appropriations for this Contract are included in the Budget Control 012 FY 2022-23 Budget.
The proposed Contract includes a provision stating that the Contract is subject to, and contingent upon, applicable budgetary appropriations approved by the Board for each fiscal year during the term of the Contract. If such appropriations are not approved, the Contract may be immediately terminated without penalty to the County.
