RECOMMENDED ACTION(S):

 

 

1.

Approve the Memorandum of Understanding with the Children and Families Commission of Orange County for the provision of Public Health Nursing Services in the Bridges Maternal Child Health Network, in the amount of $1,600,000 for the period of July 1, 2012 through June 30, 2013, and in the amount of $1,600,000 for the period of July 1, 2013 through June 30, 2014 for a total of $3,200,000.

Authorize the Health Care Agency Director, or designee, on behalf of the Board of Supervisors, to execute the Memorandum of Understanding.

 

 

 

 

SUMMARY:

 

The Health Care Agency requests approval of the Memorandum of Understanding for the Provision of Public Health Nursing Services with the Children and Families Commission of Orange County.

 

 

 

BACKGROUND INFORMATION:

 

The Health Care Agency (HCA) is requesting your Honorable Board’s approval of a Memorandum of Understanding with the Children and Families Commission of Orange County (CFCOC) for the provision of Public Health Nursing (PHN) services for the Bridges Maternal Child Health Network (Bridges Network), for the period of July 1, 2012 through June 30, 2014.  The Bridges Network is comprised of hospitals, and home based programs, and PHN services focused on developing strategic partnerships and providing responsive services to promote health early child development for young children from birth through age five (0-5) with an emphasis on prenatal through the first three years of life. The proposed memorandum includes annual of funding of $1.6 million to HCA’s Public Health Nursing program in support of four programs described below. This funding directly supports 11.5 positions in Public Health Nursing and allows HCA to leverage additional Targeted Case Management funding supporting nearly nine additional nurses.

 

Medically High Risk Infant Program provides support and intensive case management services for up to 36 months to families with medically at-risk infants who reside in Orange County.  The enrolled infants have graduated from a Neonatal Intensive Care Unit (NICU), are less than one year of age chronologically; and are at risk for developmental delays based on identified factors such as prematurity, very low birth weight, feeding or tonal problems, birth defects, and/or poor parent-infant attachment.

 

Nurse Family Partnership provides home-based comprehensive case management services to low-income women who are pregnant for the first time, under 24 years of age, and less than 28 weeks gestation.  Service priority is given to pregnant teenagers, 12 to 19 years of age.  Families are followed until the infant’s second birthday.  Nursing services may include, but are not limited to, case management, home visitation, parent education, health/psychosocial assessments, developmental screenings and referrals to community resources.

 

Perinatal Substance Abuse Services provides health care coordination, health education, and case management services to pregnant women with histories of substance abuse and/or HIV infection to ensure that their newborns are born healthy and drug-free.  To promote a stable home environment for the newborns, services include: home visitation, case management, transportation, health insurance application assistance, health and parenting education, home safety assessments, and developmental screenings. 

 

Health Access Promotion provides PHNS to work with high risk families with young children in the community to improve access to health and family support services, link families to health care homes and other services, assure that childhood immunizations are up to date and assist families in applying for and maintaining health insurance.  PHNs provide outreach to the community, home visitation and referral services for assessed health needs. As part of the program PHNs target families living in motels and shelters to address the unique needs of young children living in those locations. Services may include linkages to health services, conducting comprehensive health developmental screenings, providing families support services, and training other community providers that serve this targeted population. 

 

Each program measures specific outcomes supporting optimal growth and development, and quality of life improvement. Examples of these include: increase in the percentage of children born and remaining healthy; immunization and breastfeeding rates; linkages of families to needed medical, social, educational and other needed resources; and increase in the knowledge and skills of parents.

 

This MOU contains a mutual indemnification provision, which varies from the County standard of sole indemnification. CEO/Risk Management has approved the indemnification provisions and determined the risk to be acceptable for these services.

 

The Health Care Agency requests your Board approve the Memorandum of Understanding for Public Health Nursing Services, as reference in the recommended actions.

 

 

 

FINANCIAL IMPACT:

 

This Memorandum of Understanding is included in the Health Care Agency's FY 2012-13 Requested Budget and will be included in the budgeting process for FY 2013-14.
