RECOMMENDED ACTION(S):

 

 

1.

Approve Amendment No. 4 to amend and increase the Contract with Telecare Corporation for provision of Housing Full Service Partnership Services, for the term of July 1, 2021, through June 30, 2023, for an amount not to exceed $2,755,346, for a revised cumulative total amount not to exceed $10,345,378.

 

6.

Authorize the County Procurement Officer or authorized Deputy to execute the Amendment to the Contract as referenced in the Recommended Actions above.

 

 

 

 

SUMMARY:

 

Approval of the Amendment of the Contract with Telecare Corporation for Housing Full Service Partnership Services will provide integrated and multi-disciplinary intensive treatment and case management services to individuals living with a serious mental illness residing in permanent housing in the South Service Planning Area of Orange County.

Your Honorable Board of Supervisors (Board) approved the Master Agreement below for provision of Housing Full Service Partnership with Telecare Corporation and the below action has been executed pursuant to appropriate authorities.

Scope of Services

The Housing Full Service Partnership (FSP) will expand services to the South Service Planning Area of Orange County and will serve previously homeless adults living with a serious mental illness, who may have a co-occurring substance use disorders and who are residing in permanent housing but are at risk of losing their housing. The Housing FSP Program is a specialized field-based program that effectively engages individuals who have come from homelessness, and provides intensive and comprehensive treatment and support that focuses on preventing loss of housing while increasing housing sustainability. The program will provide services that are recovery oriented and will include intensive case management, crisis intervention, on-site housing support, transportation, medication support, individual, family and group therapy, co-occurring services, vocational and educational support, linkage to financial benefits/entitlements, family and peer support and development of supportive socialization, meaningful community roles, money management skills or coordination of representative payee services. 

 

Performance outcomes for the Housing FSP are measured by state-mandated outcome measures. The Housing FSP evaluated changes on outcomes related to days homeless, days psychiatrically hospitalized and days incarcerated by comparing functioning in the 12 months prior to enrolling in the FSP to function during the fiscal year being evaluated.

Housing FSP also has outcome measures related to housing sustainability. The Housing FSP evaluated changes on outcomes related to evictions and lease violations and maintain housing for a year of longer.

The program contributes to the overall System of Care by providing housing sustainability services to individuals who came from homelessness, which address barriers to remaining successfully housed in permanent housing including Board-approved housing developments.

 

HCA requests the Board approve the amendment to this Contract with Telecare Corporation as referenced in the Recommended Actions above.

 

 

 

FINANCIAL IMPACT:

 

Appropriations for this Contract will be included in the Budget Control 042 FY 2021-22 Budget and will be included in the budgeting process for future years..

 

 

Should services need to be reduced or terminated due to lack of funding, the Contract contains language that allows HCA to give a 30-day notice to either terminate or renegotiate the level of services to be provided. The notice will allow HCA adequate time to transition or terminate services to clients, if necessary.
