RECOMMENDED ACTION(S):

 

 

1.

Approve Master Agreement with various providers for provision of On-Site Psychiatry and Telepsychiatry Services for Correctional Health Services Programs for the term of April 1, 2023, through March 31, 2026, for an aggregate amount not to exceed $22,673,460, renewable for two additional one-year terms.

 

2.

Approve the selection of and individual Contract with Bridge Telehealth for provision of On-Site Psychiatry and Telepsychiatry Services for Correctional Health Services Programs for the term of April 1, 2023, through March 31, 2026.

 

3.

Approve the selection of and individual Contract with Lavaca Street Ventures, LLC for provision of On-Site Psychiatry and Telepsychiatry Services for Correctional Health Services Programs for the term of April 1, 2023, through March 31, 2026.

 

4.

Approve the selection of and individual Contract with Orbit Health for provision of On-Site Psychiatry and Telepsychiatry Services for Correctional Health Services Programs for the term of April 1, 2023, through March 31, 2026.

Pursuant to Contract Policy Manual Section 3.4-114, authorize the County Procurement Officer or Deputized designee to exercise a contingency contract cost increase, not to exceed a total of 10 percent of the Contract amount for the first year of the Contract, for the entire term of the Contract, including renewals, and within the scope of work set forth in the Contract. The use of this contingency contract cost increase is subject to approval requirements established by the County Procurement Officer.

 

7.

Authorize the County Procurement Officer or Deputized designee to execute individual Contracts with various providers as needed throughout the term of the Master Agreement.

 

8.

Authorize the County Procurement Officer or Deputized designee to execute the Master Agreement and individual Contracts with Bridge Telehealth, Lavaca Street Ventures, LLC, Orbit Health and Pamoja Advanced Nursing Practice, Inc., as referenced in the Recommended Actions above.

 

 

 

 

SUMMARY:

 

Approval of the Master Agreement and individual Contracts with various providers for provision of On-Site Psychiatry and Telepsychiatry Services for Correctional Health Services Programs will ensure Telepsychiatry and on-site mental health services are provided to persons in the County's custody at its detention facilities.

 

 

 

BACKGROUND INFORMATION:

 

The County of Orange (County) is mandated to provide medical care to persons in the County's custody pursuant to State of California Penal Code Sections 4011 and 4015 and Government Code Section 29602. The Health Care Agency's (HCA) Correctional Health Services (CHS) division is the designated provider of health care to adult persons in custody on behalf of the Orange County Sheriff's Department. In 2022, CHS was responsible for providing mental health services to an average daily population of 1,544 inmates with a mental health diagnosis. These services include, but are not limited to, psychiatric evaluation, development of treatment plans, medication management and crisis intervention and management. HCA has a continuous open recruitment for psychiatrists and mental health nurse practitioners, but recruitment for these classifications has proven to be difficult due to the competitive market and required lengthy and extensive background process.

 

On September 8, 2022, HCA released a Request for Application (RFA) for Psychiatry and Telepsychiatry Services for CHS. There were four respondents to the solicitation, all were deemed qualified.  Program staff evaluated applications for Bridge Telehealth, Lavaca Street Ventures, LLC, Orbit Health and Pamoja Advanced Nursing Practice, Inc., and recommended to all four providers award. The Orange County Preference Policy is not applicable to these Contracts.

 

While the average daily population of the Orange County Jails was reduced due to COVID mitigation efforts since 2020, the number of individuals requiring mental health services has remained consistently high since 2018, with an increasing trend in the acuity of the illness of the patients. The change in population and needs creates a demand for additional resources. In order to continue meeting mental health care mandates and service demand, augmenting existing onsite services with expanded hours for psychiatry and telepsychiatry services are required.

 

Scope of Services

In 2022, CHS was responsible for providing mental health services to an average daily population of 1,544 inmates with a mental health diagnosis within the Orange County Jail system. This number represented a percentage of approximately 48% of the total jail population requiring mental health servicesThis Master Agreement is established to provide both telepsychiatry and on-site psychiatric contract personnel for patient care services within Adult CHS. These services include, but are not limited to, psychiatric evaluation, development of treatment plans, medication management and crisis intervention and management for this patient population. 

 

Performance Outcomes

The goal for this agreement is to enhance contracted staffing services to ensure continuity of mandated patient care services. Contractors are expected to provide California licensed board eligible and board certified prescribers to meet the County’s needs within this agreement.

 

HCA staff have conducted due diligence on the Contractors. Reference checks were satisfactory and completed with the selected Contractors. HCA staff has verified there are no concerns that must be addressed with respect to Contractors’ ownership/name, litigation status or conflicts with County interests. 

 

HCA requests the authority to add new providers without returning to the Board for approval as referenced in the Recommended Action. Returning to the Board for approval of new providers may result in a delay that would compromise the ability to provide timely mental health services to inmates diagnosed with a mental health diagnosis. The ability to add new, qualified providers will provide the flexibility to quickly address staffing needs and is time sensitive due to the instability and needs of the at-risk population served under the Master Agreement.

 

The proposed Contracts with Bridge Telehealth, Lavaca Street Ventures, LLC, Orbit Health and Pamoja Advanced Nursing Practice, Inc., do not currently include subcontractors or pass through to other providers. See Attachment F for Contract Summary Forms.

 

These items are coming to the Board less than 30 days prior to the start of the Contracts. CHS has accelerated the start of this new contract cycle to ensure seamless continuity of services in order to augment contracted providers available to meet the current and anticipated need of the Program. The new agreement increases available contractors in order to attract and retain the volume of licensed clinical professionals needed to support mental health care services. These agreements will supplement current CHS staff in order to ensure provision of  mandated care to individuals in the Orange County jail system.

 

HCA is requesting that the Board approve the Master Agreement and Contracts with Bridge Telehealth, Lavaca Street Ventures, LLC, Orbit Health and Pamoja Advanced Nursing Practice, Inc. for the provision of On-Site Psychiatry and Telepsychiatry Services as referenced in the Recommended Actions.

 

 

 

 

 

FINANCIAL IMPACT:

 

Appropriations for these Contracts are included in Budget Control 042 FY 2022-23 Budget and will be included in the budgeting process for future years.

 

The proposed Contract includes provisions allowing HCA to terminate the Contract, reduce the level of services, and/or renegotiate the levels of services provided, as necessary. This includes a notice that allows HCA adequate time to transition or terminate services to clients, if necessary.
